Packets of forms are available for sale to be used in filing a new case or modifying a case in either the Family or Civil courts. The forms included in the packets have instructions for filling out the forms and which forms are required by the judiciary to be filed for a particular type of case. Documents should be downloaded, filled out, signed and printed in order to file with the Clerk's Office.
Each of the following packets consist of individual forms which have been approved by either the Florida Supreme Court or Florida’s 9th Circuit Court. Additionally, each packet contains instructions on how to file your documents with the Orange County Clerk of Courts. Upon completion of your purchase, the packet(s) will be available for you to download.

Petition to Determine Paternity and Other Relief (Court Packet #107)
This packet should be used if the following criteria applies:
1) There is no other court order addressing paternity, parental responsibility, time-sharing, custody, visitation, or child support concerning the child(ren).
2) You are either the child’s mother or biological father.
3) The parties were not married to each other when the child was born.
4) The child has lived in Florida for at least 6 months or since birth if younger than 6 months.
5) At least one parent lives in Florida or the child was born in Florida.

Petition for Temporary Custody by Extended Family Member (Court Packet #108)
This packet should be used by an extended family member to obtain temporary custody of a child or children pursuant to Chapter 751, Florida Statutes. This form should not be used if you are a parent seeking to establish parental responsibility or time-sharing with a child or children.
An extended family member is a relative of a minor child within the third degree by blood or marriage to the parent; OR the stepparent of a minor child if the stepparent is currently married to the parent of the child and is not a party in a pending dissolution, separate maintenance, domestic violence, or other civil or criminal proceeding in any court of competent jurisdiction involving one or both of the child(ren)’s parents as an adverse party.
